What to bring to your first visit?
We work hard to ensure that your child receives prompt medical care in a safe, comfortable, child-friendly office setting.
To help expedite your child’s first office appointment, we ask that you please bring your child’s immunization records and be prepared to complete any paperwork regarding your family medical history, your child’s medical history, and insurance information.

Patient Portal
Log into your Patient Portal for your health information.
The My Kid’s Chart patient portal allows you to view your clinical summary and is a way to conveniently directly communicate with the nurse and doctor.
The portal allows you to view your visit summary, view your medications, ask general questions for your nurse or doctor, request medical records, prescription requests, and update basic demographics.
CHADIS
Helotes Pediatrics is now offering enrollment in CHADIS, an online tool for the completion of screening forms and questionnaires.
CHADIS helps you share important information with your provider. CHADIS can be used by patients, parents, teens, grandparents, caregivers or teachers. All answers you enter are confidential and are only seen by your provider. Using CHADIS, you can share your concerns and complete questionnaires to help the clinician evaluate health, behavior and development.
Examples of forms include Developmental (ASQ-3) and Autism (MCHAT) screens, ADHD questionnaires
(Vanderbilt), Lead and TB risk screening and Asthma monitoring (Asthma Control Test). These forms can be completed online prior to the appointment via personal computer, tablet or mobile phone.

You will be asked to select the type of visit.
Please login to CHADIS before every visit and complete the appropriate questionnaires. Forms will be
available for Well Child Visits as well as certain conditions if pertinent to your child, including ADHD,
Anxiety, Depression and Anxiety.

On the day of your child’s first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office.
